RATES AND INSURANCE:

Individual counseling services is $90.00 - $110.00 per 50 minute session; $120.00 per 90 minute session.
Couple counseling is $130.00 per 50 minute session and $160.00 per 90 minute session.

Family counseling is $150.00 per 50 minute session and $170.00 per 90 minute session.

INSURANCE:
I am contracted (in network, preferred provider) with most insurance companies. Please notify me of what insurance you have. Also, upon your request, I will provide a Superbill for the counseling service that you can submit to your insurance company for possible reimbursements. Please note that I do not complete any insurance paperwork. I do accept most Employee Assistance Programs.

Many insurance plans reimburse for some portion of psychotherapy. Please direct questions about reimbursement amounts and timeliness to your insurance company. Questions to ask your insurance provider regarding your coverage might be:
1. Does my insurance provider or employee benefit plan cover mental health?
2. Do I have a deductible and has it been met?
3. What is the number of sessions per year that are covered and what percentage of each session is covered?
4. If mental health provider is not in network with my insurance, does my plan pay for out of network providers?
5. Does my insurance company accept Superbills?
6. Is approval or a referral from my primary care physician required?
7. Is a diagnosis required?

Payment is due at BEGINNING of session and I accept cash, check, or credit cards. For payment methods, please click on the Make Payment link above.

CANCELLATION POLICY: If you need to cancel an appointment, please call 970.587.4963 at least 24 hours in advance. If cancellation occurs less than 24 hours before appointment you will be charged a $50.00 fee.
